Professionals

Professionals can make referrals to council Early Help Services in several ways:

  • Online service request form for families from conception to 19 years (via the button at the end of this webpage).
  • Call the Early Help Unit on 0115 804 1248 (especially if the referral is considered urgent or complicated, or you just need advice).

How to complete the online Request for Service form

  • This online form is not for parents or carers. If you are a parent or carer and would like to refer yourself to the service, please use the email addresses or telephone numbers listed on this web page.
  • When completing the form, please make sure you have the consent from the family to make the referral prior to filling in the form.
  • Please add details of all the children in the household if known.
  • This guidance document [PDF] details the information in the form that you will need and provides examples of the information we will require under a number of outcome areas.
  • If you need help to complete the form please contact the Early Help Advisors in the Early Help Unit.

What will happen next?

Once you have submitted the form, it will be sent to the appropriate team, and you will be notified of the outcome within 10 working days.

We will keep the referrer updated if the family then accesses a council Early Help Service or intervention. We will also support referrers to identify additional support for the family if needed.
