Health and care integration
The social care reform will enable us to develop our integrated health and adult social care system by working with our partners to devise and evaluate new models of care that meet the adult social care and health needs of people in Nottinghamshire.
We will create focused action plans to ensure positive health outcomes for our residents and we will test and assess innovative approaches to emerging health needs. Best of all, the key aim is for the health and care systems to hold greater accountability, minimise bureaucracy and move towards more integrated services that improve outcomes for people and reduce inequalities in their care.
