Submit a nomination for the Nottinghamshire Outstanding Achievement 4Uth Award 2026

  • You must have permission of the person you wish to nominate before sharing their data with us. As well as the permission of their parent/guardian. 

  • Name, date of birth, address and school attended for your nominee. 

  • Name and contact details of the nominee’s parent/guardian. 

  • Name and contact information for all nominators (if there is more than one). 

Eligibility

To be nominated a young person must fulfil the following criteria: 

  • Be a resident of Nottinghamshire County Council  

  • Age between 11-19 (up to 25 years with a disability) 

  • Have achieved at an outstanding level within their own personal boundaries or has made an outstanding contribution to their community. 

  • The young person/s and their parent or guardian (if aged under 18) gives consent to be involved in media coverage relating to the award. Please note images and video footage will be circulated on our social media platforms following the nominations. By agreeing to take part you are agreeing to this. 

You will need

  • Name and contact details of the nominee’s parent/guardian. 
  • You will be asked the following three questions: 

 

    1. What has this young person or group achieved that you would like us to consider for an award? (You will need to write a minimum of 250 characters) 
    2. Please tell us more about the impact you feel they have made in their personal life or within their community. (Please be specific where you can. You will need to write a minimum of 250 characters) 
    3.  Why do you think they should win the 4Uth award? 

 

Media Consent 

You must have discussed and confirmed consent with the nominee and their parent/guardian in relation to the use of any media images by NCC. Please obtain this before starting your nomination.  

You will need to provide yes or no answers to identify their consent to the following statements: 

  • Nottinghamshire County Council’s digital channels – websites and social media sites (e.g. Facebook, Twitter, Instagram)
  • News releases to be sent to a variety of media including local print and broadcast media?  
  • Printed publicity or promotional literature (e.g. leaflets, posters, magazines)  
  • Digital channels of organisations delivering services on behalf of the Council (e.g. Inspire)? 
  • Permission to use full name with your image/recording?  
  • Details of the person featured in the image/recording?  
  • Does their parent/guardian agree to having the young person's details and photograph included in press releases? 
  • Does their parent/guardian agree to the young person taking part in media interviews? 
  • Does their parent/guardian agree to the nomination being forwarded to other partners authorised by Nottinghamshire County Council for other awards?

What will happen next

You will be taken to the online form to submit your nomination. 

You will need to complete a separate form for each nominee.
