Reablement

Reablement is free short-term support to help you regain skills and independence at home after an illness, injury or hospital stay, or when everyday tasks become more difficult.

Could reablement help me?

Reablement helps you do as much as possible for yourself, with support and encouragement from our staff.

Reablement may help if you:

  • have recently been discharged from hospital
  • are recovering from an illness or injury
  • are finding everyday tasks more difficult
  • want to regain skills, confidence and independence at home

You must live in Nottinghamshire to receive reablement support from Nottinghamshire County Council.

Reablement is part of the Maximising Independence Service.

How reablement can help

Reablement can help if you are finding everyday tasks at home more difficult. It can help you to:

  • get dressed and wash independently
  • move around your home safely
  • develop daily routines
  • build confidence
  • learn new skills
  • regain skills following an illness, injury or hospital stay

Staff may also suggest different ways of doing things or provide specialist equipment to help you manage safely and independently.

Most people who complete reablement do not need ongoing home care. Those who do often need less support than before.

Reablement works best when people are involved in working towards their goals.

What to expect

Your first visit will include an assessment to understand:

  • what you can already do independently
  • what support you need
  • what goals you would like to achieve

You will agree a support plan and goals with your reablement worker.

Staff will support you to work towards your goals and review your progress regularly. As you become more independent, your support will gradually reduce.

How long support lasts

Support may last from a few days to a few weeks, depending on your needs and progress. It can be provided for up to six weeks.

It will end when you no longer need reablement or have progressed as far as possible.

How much reablement costs

There is no charge for reablement for up to six weeks.

If you still need care and support when reablement ends, ongoing services may be chargeable.

If this happens, we will discuss your options. A financial assessment may be needed to work out how much you need to pay towards your care.

What happens when support ends

Most people regain enough independence to manage without ongoing home care after reablement.

If you need further support, we will discuss the options available and help you access other services. This may include:

  • community support
  • voluntary organisations
  • health services
  • longer-term care and support

Temporary home care may also be available while longer-term arrangements are being made. This support may be chargeable.
