Help to stay living at home

If you are struggling with everyday tasks (for example getting dressed or washed), if you have been discharged from hospital or have had an accident or illness, there are a number of services that can support you to stay living at home. 

Meals at home

Our meals at home service supplies fresh and nutritious hot and frozen meals direct to your home to help you stay living independently.

Equipment and adaptations 

Some simple equipment is available or adaptions may be needed to your home. 

Reablement

We offer a short term service called 'reablement'. This service is free and carers come  into your home to help you find the best way of doing tasks that you are finding difficult. Support is gradually withdrawn as your independence returns.

Care and support centres

Our care and support centres provide short term support to older people who have a carer emergency, need respite care, have been discharged from hospital or had a fall or fracture.

Leaving hospital 

If you are leaving hospital there are some services that can support you.

Home care and personal assistants

For people who need longer term support, there are details about home care providers, personal assistants and other services available on www.nottshelpyourself.org.uk.

Some of these services are available without a care and support assessment and others you will need an assessment. Please contact us on 0300 500 80 80 where an advisor can help your with your options and may complete a referall.
