Request for a further standard authorisation
What is this form for?
If a DoLS granted Standard Authorisation is due to come to an end and you believe the person is still being deprived of their liberty you can complete this online form to request a Further Standard Authorisation.
This must be submitted to the DoLS team at least 21 days before the expiry date of the current Granted Standard Authorisation.

Once you have completed the form you should download a copy for the relevant person's records and our DoLS team will contact you as soon as a Best Interests Assessor has been allocated.
