Healthy Family teams are community based. They work with children and young people aged between 4 -19 attending schools in Nottinghamshire and this includes children and young people regardless of their education provision living in the County.
The service aims to improve health outcomes for local children and young people. They do this by assessing health needs, delivering public health messages and offering targeted interventions including the National Child Measurement Programme, support for emotional health and wellbeing, reduction in teenage pregnancy rates, reduction in prevalence of sexually transmitted infections, substance misuse and smoking.
The service works in partnership with other agencies and professionals. They have a role to play in managing the communication between health and education so that children and young people can achieve positive outcomes and reach their full potential.
The teams are based in various health locations across Nottinghamshire. The team offer health appointment sessions to all secondary schools where young people can get confidential advice, support on a variety of health issues and targeted interventions. The service also provides appointment sessions with a member of the Healthy Family Team for primary age children and their parents.
