He was the son of George Henry and Phoebe Smith and the brother of George Henry, Oliver an Gertrude Mary Smith. In 1911 they lived at 65 Broomhill Road Bulwell Nottingham. Also living at 42 Newcastle Street, Bulwell, later of 146 St. Ann's Well Road, Nottingham.
In 1911 he was a miner (knocking off).

Private Theophilus Smith, served with the 1/7th Battalion Sherwood Foresters (Nottinghamshire & Derbyshire Regiment) (Robin Hood Rifles), he was killed in action on 13th October 1915, having no known grave his name is commemorated in the Loos Memorial.
