Day and Community Support services
What are day and community support services?
These are services for people with a physical impairment aged 18-65, aimed at increasing your independence in the community.
They cover a wide range of things such as:
- confidence building
- health and well-being
- decision making
- accessing education, employment and training
- practical life skills
- using your local facilities e.g. leisure centre
- finding suitable accommodation
- getting advice and information
- signposting to direct payments
- meeting personal assistance needs.

How do I get the service?
Get in touch with the Adult Social Care and Health's Customer Services Centre. Someone will talk to you about whether you can receive the service. We call this an assessment.

Where do I go to get the services?
This depends on the type of service or help you need. It might take place in:
- your own home
- your community e.g. your local college
- our own specialist service bases. These are in Newark, Mansfield, Worksop, Sutton in Ashfield and West Bridgford.

If I have to travel, how do I get there?
We encourage you to make your own way to places. However, should you need transport we will arrange it in our own vehicles. We will carry out an assessment to see whether this is necessary.

How much will all this cost me?
The only charges we make are for:
- any transport we provide
- any food you buy in our service bases
- any community activities you may wish to take part in.

I’m interested! What happens next?
We agree with you the things we can help with, how often you need to use the service and how long you need it for.
One of our staff will meet with you regularly to see how you are getting on.

What if there are any problems?
Please tell any of the staff straight away. If you are still unhappy with the service you receive you can make a formal complaint. Contact the Customer Relations Service.
