Hospital discharge scheme - Volunteers needed

What is the Hospital Discharge Scheme?

The Hospital Discharge Scheme provides low level support to patients and their carers when discharged from hospital.

The Hospital Discharge Scheme has paid staff and volunteers.

We need more volunteers - could you help?

Would you like to help people in your community, especially the elderly?

Do you have time to spare?

Are you a caring person who is able to listen?

Would you like to gain experience in social care, to assist in future job prospects?

What do volunteers do?

Volunteers provide emotional and practical support to service users who have little or no family support. They are the link between hospital and home.

We provide short term support to people recently discharged from hospital.

As a volunteer, you will be a member of a very supportive and caring team.
