What is the Hospital Discharge Scheme?
The Hospital Discharge Scheme provides support to patients and their carers when discharged from hospital. It is run by paid staff and volunteers who will visit you in your home and provide a range of help.
It is available in the Ashfield and Mansfield districts, and some of the surrounding areas.
It may be that you are well enough to leave hospital, but not yet well enough to manage your daily tasks. Or it may be that you will need help for a longer period, but we have not had time to arrange all the services you will need.
The Hospital Discharge Scheme can provide short term help, until you are able to manage for yourself, or until ongoing services have been put in place.
What help can I get?
The Hospital Discharge Scheme can help you with things like:
- visiting you on the ward before your discharge
- paying your bills
- making you a warm drink and snack
- doing your shopping
- collecting your prescription
- doing light domestic chores
- feeding your dog.
Volunteers do not provide personal care or replace paid services.
Transport may be available in special circumstances.

This is a free and confidential service.
Who are the volunteers?
All our volunteers are police checked and carry photo identification.
