Referrals are taken from mental health professionals, usually Care Programme Approach (CPA) care co-ordinators. If you have any queries about whether a referral is appropriate or not, please feel free to call us.

To commence a referral, we require copies of the following CPA forms:

  • Initial Assessment
  • Care Plan
  • Risk Management form.

Referral information is taken to the centre's weekly referral meeting, at which a worker is allocated to follow the referral through. The allocated worker then contacts the referrer to obtain further relevant information and agree on how best to engage with the client. We try to make initial contact with the referred client within ten working days of receiving the formal referral.

At this point, the client will be invited for their first formal visit to the centre, at the end of which, if appropriate, attendance arrangements may be agree.

The Middle Street Resource Centre aims to provide a service for people with substantial or enduring mental health difficulties. Where an individual's need is primarily for short term counselling, it may be that this need is best met elsewhere.

Whenever we consider a referral to be inappropriate, we will do our best to suggest alternative sources of support.
