Day and Community Support Services
These services help people with a physical disability to develop the skills they need to become more independent in the community.
This scheme offers a specialised door-to-door travel service for people who are unable to use public transport.
We can offer money to people so they can arrange their own care and support at home – instead of the Adult's Department providing it. This is called a direct payment.
A Disabled Facilities Grant is to help towards the cost of adapting your home. You may be entitled to a grant if you have difficulty managing daily living tasks at home.
These badges allow people who have severe walking difficulties, or who are registered blind, to park their cars near shops, stations and other facilities.
We have a fund for grants to help pay for a wide range of items. These items should improve a disabled person’s lifestyle and independence.
Disabled people - we provide several options to enable people with a physical disability to have a break. The Fund for Disabled People (see above) may be able to help pay for your holiday.
Carers – We an also offer support to carers to obtain respite care.
Home Care is someone coming into a persons home to help with personal care (dressing, washing, etc.) or practical tasks (laundry, shopping, etc.).
If you need re-housing you should contact your local district council and/or housing associations (see Phone Book for contact details).
This is a delivery service of ready-made meals, direct to people's homes.
The Occupational Therapy service visits people at home to look at the practical difficulties they are having and help find solutions. This can include advice and information on the best way to do things, how to get adaptions to your home and the loan of specialised equipment.
Residential and Nursing homes are now called Care homes. If people are unable to continue living in their own home and need residential care, the Adult Social Care and Health Department can arrange this. It can be either short or long-term in a care home or, wherever possible, allowing you to live independently in one of our extra care scheme homes.
This scheme is for people who have particular mobility difficulties. The permit allows vehicles into pedestrianised areas within the County.
This scheme offers disabled people a range of opportunities to gain the skills and experience necessary to obtain long-term employment. We also support them once they have found a job. Advice for employers on employing disabled people is also available.
The Welfare Rights Service provides free, independent information, advice and advocacy on welfare benefits.
