Nomination tips
- The forms for this year's Sports Review are slightly different
- This guide should help you make a more successful nomination
- Don't forget that Club of the Year has a different application form and includes it's own guide to completion.
General:
- Only official application forms or online entries will be accepted.
Nominee information:
- It is important to include the full address of the nominee including a postcode
- Please include a telephone number with area code
- An e-mail address is also useful if you have one
- You only need to fill in the age section if the nominee is under 19
- You must fill in the main sport for which you are making the nomination.
Nominator information:
- It is important to include the full address of the nominee including a postcode

- An e-mail address is also useful if you have one.
Award category:
- You can only nominate for one category per nomination form so only tick one box.
Achievements:
- Each achievement should have a date and level included unless it is a Roll of Honour or Volunteer nomination
- It is not necessary to complete all five sections for achievements if you cannot think of five
- Be as detailed about the achievement as you can. For example: A bronze medallist at the World Championships lifting 87.5kg November 2006 is better than International bronze medal winner November 2006.
Other information:
- Use this space to add anything else to the application (you cannot attach extra sheets). This area should be particularly useful for Roll of Honour and volunteer nominations
- You could include things like how many hours a week are spent volunteering, how many people the nominee works with, qualifications obtained, hours spent training, variety of roles or tasks completed. Anything you think is relevant for accomplishments in the past year.
Signature:
- All nomination forms must be signed by the person completing the form.
