If you think you need care and support from Nottinghamshire County Council you will need a care and support assessment [PDF] to decide if you are eligible.
The decision about whether you are eligible is based on the information you have provided in the care and support assessment and whether:
- your needs arise from, or are related to, a physical or mental impairment or illness
- as a result of your needs, you are unable to achieve two or more outcomes specified in the Care and Support (Eligibility Criteria) Regulations 2014. The regulations are set out in our factsheet [PDF].
- as a consequence there is, or is likely to be, a significant impact on your wellbeing
