How to make an application
If someone needs to be deprived of their liberty immediately, an ADASS Form 1 should be completed. All pages excluding the ‘request for an extension to the urgent authorisation’ should be completed. The first part of the form is to request a standard authorisation, the second part (after the ‘Religion or Belief’ section) grants an urgent authorisation which lasts for 7 days.
In exceptional circumstances you can complete the third part of the ADASS Form 1 (Request for an extension to the Urgent Authorisation) to apply for an extension (up to seven days) to an urgent authorisation.
A standard authorisation can be reviewed at any time and is made by using ADASS Form 10 (Review). This form can also be used to advise of a change in circumstances, e.g. the person has moved, or has passed away.
If a person passes away whilst under a standard authorisation (the DoLS has been assessed and granted) then a form 12 needs completing and sending to the DoLS team, and to the Coroner. This also applies if the person is discharged to hospital and subsequently passes away in hospital.
Request to renew a DoLs authorisation
If a DoLS authorisation is due to come to an end, and the managing authority believes they are still depriving the person of their liberty, an ADASS Form 2 should be completed. This must be received into the DoLS team at least 21 days before the expiry date of the current standard authorisation.
If the managing authority believes there has been a change and the person is no longer deprived of their liberty, or if they believe any of the conditions should be reviewed, then an ADASS Form 10 (Review) should be completed and should be received into the DoLS team at least 21 days before the expiry date of the current standard authorisation. An ADASS Form 10 can be submitted to the DoLS team at any time during the authorisation.
